Housing First Blended Case Management


Housing First Blended Case Management (HF-BCM) Program assists individuals who have been chronically homeless and in need of obtaining and maintaining housing.  We provide services to eligible adults 18 and older who also may be struggling with mental and/or co-occurring disorders.  Tenant based Rental Assistance (TBRA) Funding will not be available to individuals after age 62.

Services

HF-BCM offers housing referrals, help with housing costs (move in costs, security, deposits, rental applications, rental assistance), integrated treatment for mental health and/or substance use disorders, connection to other community resources, service planning and coordination, and empowerment and self-help.

Eligibility

Those eligible for HF-BCM are single individuals aged 18+ who are experiencing chronic homelessness and diagnosed with or demonstrating behaviors indicating a mental health disorder.

Referrals & Intake

All referrals must include a completed HF-BCM referral form, as well as the referral form completed in HMIS.  Referral will not be complete without insurance verification of eligibility.  A referral may come from a street outreach team, the Bucks County Emergency Homeless Shelter or Bucks County BH/DP.

Locations

HF-BCM serve clients in the community, at the homeless shelter, where clients work or live and in our offices.  Our offices are located in Langhorne and Quakertown.
