We provide varying levels of service to meet your needs. You choose the level that fits.
We provide you with general information about aging and services available to help you and/or your loved one.
Our team conducts and evaluation of the physical, emotional, financial and social functioning of each client, and we review any relevant medical, financial or legal issues that may exist.
- Care Planning
We develop a Care Plan of recommended services and help you and/or your loved one understand the benefits of various types of care and services available.
- Care Coordination
Our team obtains services identified on the Care Plan and visits and monitors clients regularly.

- Home care services
- Health management
- Health insurance
- Medication assistance programs
- Social activities and/or companionship
- Legal assistance
- Financial assistance
- Veteranís benefits
- Safety and security
Our services are open to all residents of Bucks County.
- $80 per hour
- Mileage billed separately at $0.45 per mile
